WiffleBall Waiver

WAIVER ASSUMPTION OF RISKS. AND RELEASE OF LIABILITY

Above named being allowed to participate in The Rancho Grande Wiffle Ball Tournament for Alzheimer’s (the “Tournament”), the undersigned releases and agrees to fully indemnify Rancho Grande Cantina, Inc., Crestwood Center, Alzheimer’s Association-Heart of America Chapter and all of their respective sponsors, officers, directors, shareholders, agents and employees, which are hereinafter collectively referred to as “Releases”, from any and all liability to the Participant, and his or her heirs for any and all claims, demands, losses, damages, or expenses, including attorney’s fees, on account of any injury suffered by Participant which is caused or alleged to be caused in whole or in part by the negligence of any Releases , including but not limited to the maintenance or use of the grounds on which the Tournament is held.
The undersigned acknowledges that participating in the Tournament including the use of the grounds on which it is held involves a risk of injury to Participant not only as a result of the participant’s actions but also from the actions of others as well as the condition of the grounds and the facilities used in the Tournament. The undersigned assumes all such risks and accepts personal responsibility of any injury or damage suffered by Participant during the Tournament.
I HAVE READ THE ABOVE WAIVER AND RELEASE, UNDERSTAND THAT I AM CREATING LEGALLY BINDING RIGHTS BY SIGNING IT AND SIGN IT VOLUNTARILY.
